Pomelo Care is building the first at-scale, evidence-based pregnancy and newborn care platform, transforming outcomes for pregnant people and babies. Our technology-enabled care platform engages patients early, conducts continuous risk assessments, creates a personalized care plan and team for each patient, and delivers coordinated virtual care and care management throughout pregnancy, NICU, and in the postpartum year to improve outcomes.
Pomelo manages individual risk factors for poor pregnancy outcomes (e.g., diabetes, hypertension, behavioral health, substance use disorder, social determinants, etc.) to reduce preterm births, NICU admissions, c-sections, and maternal mortality and morbidity. We also support families whose babies are in the NICU to help them care for their newborns, prepare for timely discharge, and confidently transition to home care with 24/7 telemedicine access to the Pomelo care team.

Key responsibilities of the role
Your North Star: Provide superb care for Pomelo patients in a telehealth setting.
We are hiring a team of creative, passionate, and empathetic clinicians eager to improve pregnancy and neonatal outcomes. You will provide virtual care before and during pregnancy, as well as postpartum. Care is provided in one-on-one sessions, as well as in groups. You will work closely with other members of the care team to provide coordinated, team-based care with proactive follow-up, including communicating with the patient’s in-person providers. You will participate in learning what is important to Pomelo’s patients and innovating about how we can best serve their needs.
In the role, you will:
- Provide timely, evidence-based patient-centered care via text, phone and video chat formats
- Facilitate virtual groups for various populations: fertility, pregnancy, antepartum, postpartum and others as needed
- Both use and help develop iterative versions of Pomelo technology platforms for communication and patient care
- Document patient encounters in an efficient and timely manner
- Participate in the quality and safety programs that are part of our continuously-learning health care practice, including case conferences, journal clubs, structured self-assessment and reflection opportunities, invited guest lectures, and more
- Collaborate with the interdisciplinary team to maximize care coordination and to expand and develop model as we grow
- Innovate about how Pomelo can best engage patients and meet their needs
- Actively foster and participate in a culture of equity, diversity, and inclusion

Who you are
- A holder of an unrestricted CNM, NP, or PA license to practice in the US
- Minimum 1-2 years experience of clinical care in women’s health and the care of pregnant and postpartum patients
- Experience in outpatient or home-based management of higher-risk patients
- A passion for optimizing evidence-based care and outcomes for pregnant people and newborns
- Experience and comfort with telehealth care delivery
- Facility and flexibility in using and helping advise about development of tech platforms for patient care
- A growth mindset with the ability to approach process change and ambiguous situations with enthusiasm, creativity, and organization
- Ability to thrive in a fast-paced, metric-driven environment
- Phenomenal interpersonal and communication skills
- A humble team player who always asks: how can I contribute positively?
- Bonus points for:
- Experience with group facilitation/group model care
- Experience at an early stage health tech company
- Qualification to practice in any of FL, LA, MA, MS, NC
- Licensure to practice in multiple states
