Job Description

The Role:

We are looking for a board-certified internal or family medicine physician to provide clinical care to patients using telehealth solutions. The physician will collaborate with nurse practitioners and/or physician assistants in applicable states, provide direct care to patients through telemedicine, and will report directly to the Market Medical Director. Collaboration with clinicians will include conducting patient chart audits on a regular basis, answering clinical questions and reviewing clinical protocols, and administrative oversight of documents for coordination of care. The ideal candidate should have familiarity with value-based care and be comfortable working in a novel care delivery model.  

Job Responsibilities:

  • Provide primary care and see patients using telemedicine
  • Treat and manage the health of patients (all adults and seniors) as it pertains to their wellness, urgent medical issues, and chronic illnesses with the goal of establishing a primary care relationship
  • Addressing HEDIS measures and closing gaps in medical care
  • Attention to detail, especially in medical documentation
  • Collaborate with nurse practitioners and/or physician assistants, be a teammate and leader to clinical staff in the local market
  • Support training and provide mentorship to new clinicians in the local market 
  • Support local care coordination to close care gaps and optimize outcomes
  • Participation in Heal related activities to build and test products
  • Work with Medical Directors and the Chief Medical Officer to identify opportunities to innovate and improve medical care

Requirements:

  • Current and active MD/DO license (Louisiana or North Carolina)
  • Active Licenses must be in good standing
  • Board certified or board eligible in internal or family medicine
  • Active DEA License
  • Experience in primary care necessary, with value based care preferred
  • Telemedicine experience is a plus
  • Board certification in geriatrics is a plus
  • Excellent verbal and written communication skills in English
  • Excellent technology-based tools skills (eg. EHR, Google Suite)
  • Stable Wifi connection and private working space (HIPAA-compliant)
