About the Role

We are seeking a NP with psychotherapy experience to help improve the quality of and access to mental healthcare for millions of seniors across America. As a psychiatry NP on our Clinical Care team, you will work with residents in our partner facilities to help improve mental health amongst residents and, in turn, improve their overall health and quality of life.  

Working from the location of your choice (e.g., home, office, clinic) you will be assigned a patient panel that you will treat longitudinally (i.e., you will see the same patients visit after visit) on a pre-scheduled basis. Tembo leverages technology to improve efficiency and quality through innovations we build. For instance, we integrate data from local site facilities' platforms with our EHR as well as collect patient collateral from caregivers prior to your scheduled clinical encounters.

Responsibilities 

  • Assess new residents. You will help residents known and suspected of having mental health conditions adapt to their new homes by conducting initial mental health evaluations and determining care regimens inline with their mental and overall health care goals.

  • Conduct followup appointments. You’ll use best practices in behavioral health to help your patients live healthier, high quality lives. This includes medication management, talk therapy and engaging in interventions as necessary.

  • Support urgent care incidents. You’ll perform evaluations of patients that experienced an urgent care incident to determine if any treatments or related therapeutic procedures are necessary. In addition, you may help support communities by performing crisis interventions when some patients experience mental health episodes.

  • Collaborate with other health providers. You’ll work with patients’ primary care providers, the staff at the senior living facilities, and others to make sure that your patients are receiving holistic and consistent care. You’ll refer patients to urgent care or talk therapy when necessary.

  • Serve as a clinical subject matter expertise. Provide input and collaborate with our internal teams and stakeholders on our products and services. You’ll also help define and build workflows that enable us to deliver efficient and positive experiences.

About You

Requirements

You’ll be a fit for this role if you have/are:

  • 5+ years of experience as a nurse practitioner in psychiatry and mental health, including providing psychotherapy.

  • Actively licensed as a psychiatric/mental health nurse practitioner and certified with the DEA to prescribe medication. Bonus if you are licensed in NY, TX, and/or IL.

  • Excellent assessment, diagnostic and psychopharmacological treatment skills; experience with dementia patients a plus.

  • Strong interpersonal skills when dealing with patients, physicians, and caregivers.

  • Able to work with details and independently to problem solve, collaborating as needed.

Suggested preferences

  • 10+ years of experience as a nurse practitioner with a focus on geriatric psychiatry or related field.

  • Geriatric experience or worked closely with a senior population.

  • Knowledge using the Athena Health electronic health record system.
