Our Case Manager is responsible for assessing, planning, implementing, monitoring and evaluating options and services to effect an appropriate, individualized plan for the client across the continuum of care. Using independent judgment, the case manager utilizes clinical knowledge and competence, communication skills, problem solving and conflict resolution to effectively ensure optimal client outcomes with consideration to payor requirements. Will work effectively with all health care team members internally and externally.

What you’ll do:

  • Manages both medical and behavioral health care of clients through the health care system based on the individual’s needs, utilizing clinical knowledge, communication skills, and problem solving and conflict resolution skills.
  • Uses independent judgment and discretion to address, resolve, and process problems impeding the diagnostic or treatment progress. Proactively identifies and resolves delays and obstacles.
  • Seeks consultation from physicians, clinical and other disciplines and departments as required to expedite care, monitor length of stay, and facilitate discharge.
  • Pre/Post admission member outreach to assess level of understanding of hospital stay and close any knowledge gaps, as well as discharge planning to support understanding of physician orders and prevent readmission. May also manage length of stay to ensure timely, appropriate discharge.
  • Maintains open communication with all appropriate parties.
  • Maintains strict standards for client confidentially and client related information; complies with all organizational, state and federal regulations and policy on confidentially.
  • Per URAC standards, ensures compliance with case management documentation and completes reports timely.

What you’ll need:

Required Qualifications

  • Active RN licensure required
  • Graduated from a School of Nursing or College
  • CCM preferred
  • BSN or related Bachelor’s degree preferred or knowledge at a level usually associated with a college degree and 5+ years of directly-related experience preferred.

Experience

  • Minimum of two (2) years clinical experience in an acute-care setting and 1 year complex and/or case management experience.
  • Basic computer skills (copy/cut, paste, maneuvering screens and typing) necessary.

Knowledge, Skills, Abilities

  • Excellent verbal and written communication skills essential.
  • Ability to utilize critical thinking and apply sound clinical judgment and assessment skills for decision- making.
  • Strong analytical and data management skills.
  • Strong organizational and time management skills and the capacity to prioritize multiple tasks.
  • Ability to work in a fast paced environment with often changing priorities.
  • Current working knowledge of utilization management, case management, and managed care.
  • Demonstrates the ability to be organized and efficient in prioritizing and managing assignments with minimal oversight and direction. Computer proficiency in Microsoft office and other software programs and the ability to enter and retrieve data from relevant computer systems.
