Job Description

Eternally is here to radically improve end-of-life care. This is something that impacts everyone in the world and yet, we are still using bizarrely antiquated methods of planning and preparing for it. Eternally’s mission is to remove the barriers to Advance Care Planning by providing personalized support to patients around the country.  

We are seeking a Telemedicine Clinician (MD/ DO / NP / PA) with experience in having empathetic, compassionate, and educational advance care planning conversations to help patients complete their living will/advance directive from the comfort of their own home driving our mission to enable this support for everyone anywhere at any time.

Responsibilities:

  • Ability to provide caring and educational advance care planning information via telehealth (audio/video) conversations

  • Guide conversation to action by encouraging patients (or their caregivers) to complete their Advance Directive 

  • Ability to hold up to 6 ACP conversations daily

  • Understanding of the needs of the geriatric population and the process of aging

  • Ability to communicate effectively, in verbal and written form

  • Exhibit patience under stressful conversations

  • Telemedicine Clinician (MD/ DO / NP / PA) must possess an active, current, full, and unrestricted license to practice in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Clinicians must maintain current registration in the State of licensure if this is a requirement for continuing active and current licensure in that State.

  • Graduate of accredited program for Physician Assistant

  • Graduate of an accredited school of medicine or osteopathy, Graduate of accredited program for Physician Assistant, or Graduate of accredited program for Nurse Practitioner

  • Current clinical opportunities for the Telemedicine Clinician (MD/ DO / NP / PA) are based in CA, FL, TN, IL, NJ, DE, PA

  • Hours range from 6 hours/week to full-time with flexible scheduling based on a M-Sat schedule.
